Vertigo is commonly associated with disorders of the inner ear, the vestibular system, or neurological conditions. For this reason, people experiencing dizziness are often referred to an otolaryngologist or a neurologist. However, in a significant number of cases, vertigo does not originate from the ear or the brain. Instead, it can represent a peripheral signal of cardiovascular dysfunction, particularly related to altered cardiac perfusion.

Understanding the relationship between vertigo and the cardiovascular system is essential for correct diagnosis and early intervention. The brain is an organ with very high metabolic demands and is extremely sensitive to even brief reductions in blood flow. When the heart is unable to guarantee an adequate and stable supply of oxygenated blood, cerebral perfusion can be compromised. This reduction in blood flow may manifest as vertigo, dizziness, blurred vision, lightheadedness, a sensation of near fainting, or postural instability.

In these situations, vertigo is not a problem originating in the head but rather a signal that the heart is struggling to adapt to the circulatory demands of the body. Recognising this link allows for earlier and more effective treatment.

Cerebral perfusion depends on a delicate balance between cardiac output, blood pressure, and vascular regulation. When this balance is altered, even temporarily, symptoms can appear. One of the most frequent cardiovascular causes of vertigo is orthostatic hypotension, a condition characterised by a sudden drop in blood pressure when moving quickly from a lying or seated position to standing. This drop causes a transient reduction in blood flow to the brain.

Orthostatic hypotension is particularly common in older adults, dehydrated individuals, people taking antihypertensive medications, and those with impaired baroreceptor function. Baroreceptors, located mainly in the carotid arteries and the aortic arch, are responsible for regulating blood pressure during postural changes. When their response is insufficient, cerebral perfusion may decrease abruptly, leading to dizziness or vertigo.

Another important and often underestimated cause of vertigo is cardiac arrhythmia. Both slow heart rhythms, such as bradycardia, and fast or irregular rhythms, such as tachycardia or atrial fibrillation, can reduce the efficiency of cardiac output. When the heart’s pumping action becomes irregular or insufficient, cerebral blood flow may become discontinuous or inadequate.

Patients with arrhythmia-related vertigo may experience dizziness, visual disturbances, mental fog, weakness, palpitations, or sensations of near syncope. In these cases, vertigo is not a primary disorder but a symptom of fluctuating cerebral perfusion caused by altered cardiac rhythm.

This relationship is particularly relevant in older adults. With aging, changes in vascular elasticity, autonomic regulation, and cardiac function increase vulnerability to cerebral hypoperfusion. Because symptoms are often subtle or attributed to aging, cardiovascular causes of vertigo are frequently overlooked, increasing the risk of falls, injuries, and cardiovascular events.

When vertigo is suspected to have a cardiovascular origin, management should be integrated and multidisciplinary. This includes cardiological evaluation, electrocardiography, blood pressure monitoring, assessment of hydration status, and review of ongoing medications.

Lifestyle also plays a supportive role in cardiovascular and circulatory health. An alkaline-oriented lifestyle, when approached in a balanced and evidence-based way, may support hydration, electrolyte balance, vascular function, and reduction of systemic inflammation. Adequate hydration, proper mineral intake, balanced nutrition, regular physical activity, stress management, and quality sleep all influence blood pressure regulation and cardiac efficiency, factors directly linked to cerebral perfusion.

Vertigo should therefore not always be interpreted as a disorder of the inner ear or the brain. In many cases, it represents an important warning signal from the cardiovascular system, indicating difficulty in maintaining adequate blood flow to the brain. Recognising this connection allows earlier intervention and a more effective, comprehensive approach to treatment.
